Mobile Collections

Eligibility

Mobile Collection bookings are prioritised based on clinical need, to ensure QML Pathology can continue to deliver the highest standard of care and support to our patients and healthcare practitioners.

Our Mobile Collection service is designed for patients who are genuinely unable to attend a collection centre due to medical, mobility, or residential care needs. We thank you for understanding the intended medical criteria for this service, so that we can avoid impacting availability for those who need it most.

This webform booking process currently applies to appointments in the metro area only.

Booking Process

  • The booking form allows you to select the day(s) you are available for a Mobile Collection. Our team will book the next available collection on your preferred day.
  • Appointment times are typically confirmed within 72 hours of receiving your booking request.
  • Urgent bookings are restricted to requests submitted by healthcare providers.
  • New patient requests must be submitted by the patient's healthcare provider or practitioner. Alternatively, the referrer can record a written and signed note on the patient's referral form requesting a home visit and stating the reason. Once this is on file, existing patients can book online independently for subsequent eligible visits.
  • Booking confirmation will be sent via email.
  • We may contact you regarding your booking request.
  • Rule 3 patients must present their Rule 3 card/referral at each collection. A new Rule 3 referral is required for every 6 bookings. This online form can be used to book your next set of appointments and upload the new referral.

Animals on Premises

To help us keep our collection staff safe during your Mobile Collection appointment, we ask that you provide accurate information about any animals present at the collection address.

  • Disclosure required. You must let us know whether any animals will be on the premises at the time of your appointment, including the type of animal(s) present (e.g. cat, dog, or other).
  • Animals must be secured. Any animal on the premises must be secured (for example, in another room or restrained) for the full duration of the collector's visit, unless otherwise agreed with our team in advance.
  • History of aggression. You must disclose any known history of aggression for animals on the premises. This information helps us assess the appointment safely and, where necessary, put additional precautions in place.
  • If this information changes between booking and your appointment date, please contact us to update your booking.
  • Our collectors reserve the right to decline to proceed with a collection, or to reschedule, if an animal on the premises is not appropriately secured or poses a safety risk at the time of the visit.
